Since 1995, the mission of Rural Health Association of Tennessee (RHA)is to lead the way to a healthy Tennessee through partnerships, advocacy, education, and resources. Our 700+ members include rural hospitals and clinics, school health professionals, mental and behavioral health providers, and others who work to address health disparities in rural and other minority populations.

Improving access to care, especially primary care, is a foundational pillar to improving health outcomes in Tennessee. There are three main types of facilities that provide care to rural communities: Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), and Critical Access/Acute Care Hospitals (CAHs).¹

This dashboard provides key data elements to better inform policy makers, administrators, funders, and others wishing to strengthen access to care in rural Tennessee.

The expiration of the Medicare telehealth waivers on September 30, 2025, has created significant uncertainty for providers as longstanding flexibilities have been scaled back or altered. While Congress continues to negotiate a government funding package that may reinstate some of these flexibilities, permanent statutory restrictions are now in effect layered with CMS guidance that is at times ambiguous or unclear. As a result, providers are facing complex questions regarding in-person visit requirements, billing procedures, and the applicability of policies across different care settings. This FAQ document is designed to address the most common questions CCHP has received through its technical assistance channels since October 1, 2025, and to highlight where CMS has offered clear direction, and where gaps remain.

These extensions were tied to the federal continuing resolution. With ongoing political uncertainty and many competing priorities in Congress, there’s a real risk that lawmakers may not act in time—creating a “telehealth policy cliff” that would roll back policy gains.

Proposed CY 2026 Physician Fee Schedule

Much of the telehealth community has been waiting to see what the Centers for Medicare and Medicaid Services (CMS) will be proposing in the 2026 Physician Fee Schedule (PFS). The wait was over when on July 14, 2025, the unpublished version of the 2026 PFS was released (the official version was subsequently published in the Federal Register). For those who are unfamiliar, the PFS is the vehicle CMS uses to update, change or introduce new policies that will impact the Medicare program for the following year. Unless otherwise stated, the policies typically go into effect on the first of the new year.
