		<p><em>IDHI uses digital tech to tackle healthcare and broadband disparities in Arkansas. HRSA and BEAD play crucial roles in doing that.</em></p>
<p><em>HRSA, under the U.S. Department of Health and Human Services, focuses on improving healthcare access for vulnerable groups. It supports clinics, health programs, and professional training, funded through federal appropriations.</em></p>
<p><em>BEAD, part of the National Telecommunications and Information Administration (NTIA) within the Department of Commerce, aims to expand high-speed internet, especially in underserved areas. Funded by the Bipartisan Infrastructure Law, it&#8217;s geared toward broadband infrastructure development.</em></p>
<p><em>While HRSA and BEAD share objectives, they&#8217;re funded and managed differently by distinct federal departments.</em></p>

		<p>Despite being separate entities, BEAD and HRSA complement each other&#8217;s goals. BEAD focuses on broadband infrastructure, while HRSA concentrates on health services. BEAD&#8217;s broadband expansion directly aids HRSA&#8217;s objectives, especially in telehealth. It allows remote patients access to specialist consultations and mental health services without extensive travel. Broadband growth also supports health information technology (HIT), improving care quality and efficiency in HRSA-backed facilities. For education, better internet access enables online training for healthcare workers in underserved areas, helping them stay updated with medical practices. Enhanced connectivity aids public health initiatives, facilitating data collection, tele-public health, and real-time communication during emergencies. It also boosts patient engagement through access to online records and tools for managing chronic conditions. Essentially, BEAD&#8217;s infrastructure improvements indirectly enhance HRSA&#8217;s mission, fostering a more equitable healthcare system for all, regardless of location.</p>

		<p>In August of 2010, UAMS received a grant for 102 million to provide equipment and dedicated circuits to healthcare providers and the Arkansas Department of Health. This began the Institutes drive into broadband expansion in Arkansas. In 2020, IDHI was recruited by the Arkansas Governor’s Office to design and operate a program for broadband grants to ISPs, called the Broadband Technology Opportunities Program (BTOP), for the expansion of broadband as driven by the pandemic. The project is still functioning today and growing each year until each household is served.</p>

		<p>While ARC focuses on broadband and UAMS on healthcare, ARC&#8217;s efforts in rural internet access are vital for expanding UAMS&#8217;s telehealth and education outreach. This collaboration addresses rural communities&#8217; challenges comprehensively. Since 2020, UAMS &#8211; IDHI has managed the ARC Project, allocating over $555 million to ISPs for broadband expansion, greatly impacting Arkansans&#8217; access to essential services.</p>

<p>Here’s a question we’re getting a lot right now: <em>When are the policy changes that went into effect at the onset of the coronavirus pandemic expiring?</em> Or: <em>Will these changes be made permanent?</em> The fact is, we don’t know but we’re seeing the winds of change starting to take hold. Whereas wide acceptance of telehealth was an uphill climb for many pre-pandemic, the current demand for telehealth has flung the doors wide open. Blue Cross Blue Shield in Tennessee recently announced the permanence of the temporary pandemic changes.</p>
<p><em>&#8220;In March, the mission-driven insurer began covering telephone and video visits with in-network providers. The change initially included primary care providers, specialists and behavioral health providers and was later expanded to include occupational, physical and speech therapy as well as ABA therapy services. All of these services will now be covered on an ongoing basis.&#8221; (BCBS)</em><sup>1</sup></p>
<p>Last week, Centers for Medicare and Medicaid Services (CMS) released guidelines with a statement encouraging the resumption of in-person care saying, “while telehealth has proven to be a lifeline, nothing can absolutely replace the gold standard: in-person care.”<sup>2</sup></p>
<p>CMS is mostly referring to those who have postponed non-emergency visits during the onset of the pandemic, and with some states seeing spikes in COVID-19 cases, many are wondering what this resumption of care will look like. The Mississippi Hospital Association and Mississippi State Medical Association created Prepare to Care, a campaign to inform their residents about safe treatment options… including telemedicine.<sup>3</sup></p>
<p>Health professionals and lawmakers alike are calling for telehealth expansion. Thomas Ferrante with Foley and Lardner law firm says, “Before the pandemic, Medicare fee-for-service coverage of telehealth services was extremely restrictive, with limitations for rural locations, originating sites, eligible practitioners and services, and qualifying technology. The CARES Act allowed CMS to temporarily remove these requirements under broad waivers.”<sup>4</sup></p>
<p>With national and local levels of government revisiting and revising policy, public and private sectors reviewing and restructuring operating procedures and financial matters, no one knows what the healthcare landscape will look like when we come out on the other side. But one thing is certain: there will be no return to the way it was.</p>

<p>One knock on technology is that it moves us away from making personal connections with people in our lives or communities. In many ways it’s true.  Think about the time you spend on social media sites instead of actually paying a visit to a friend or family member.  Similarly, how many times do you see people using cell phones in public places completely unaware of their physical surroundings?</p>

<h3>A Knock on the Door</h3>
<p>In medicine, physicians used to make house calls.  They’d travel the community visiting folks who could not make it into the office for an appointment. Unfortunately due to a lack of healthcare providers this isn&#8217;t really possible today.</p>

<p>However, now technology can bring people together.</p>
<p><em>Consider this story I recently heard. There was a quadriplegic rehabilitation patient who lived nearly three hours away from the only specialist in the state that could provide treatment.  The patient was not able to travel to the healthcare provider so the healthcare provider had to come to him.</em></p>
<p><em>Hearing this, a nurse practiced in Telemedicine, took it upon herself to deliver a clinical Telemedicine cart to a health center in the patient’s area of the state.  The RN also coordinated with the physician to have a video unit delivered to his office.  The patient was able to connect with the physician via a live video connection!</em></p>
<h3><strong>Knock it out of the Park</strong></h3>
<p>After the exam several good things happened:</p>

<p>Now, luckily I live in a state that benefits from a well designed infrastructure that helped make this encounter possible.  But, it was the tenacity of the providers and their commitment to bring healthcare to medically underserved areas that truly made it happen.   This tenacity and commitment is what motivated doctors to make those house calls years ago.</p>
